Overview
Use this form to apply at the counter for an NHI eligibility confirmation certificate or eligibility information notice, or for a reissue of an elderly recipient certificate. People without a My Number insurance card receive the certificate by registered mail to the household head. People with a My Number insurance card receive an eligibility information notice. Applications are accepted at the NHI and Pension Section counter and at citizen offices. Application forms and a filling example are attached.
What this form is for

- Reissue of the elderly recipient certificate
Without a My Number insurance card
- The eligibility certificate is sent to the household head by registered mail.
- If the certificate has not arrived before you need care, ask at the counter for an eligibility status confirmation form to show at the medical institution.
- People who hold one identity document that verifies identity can receive the certificate at the counter, as an exception.
- People aged 70 to 74 also receive an elderly recipient certificate.
With a My Number insurance card
An eligibility information notice is issued instead of a certificate. People aged 70 to 74 do not receive an elderly recipient certificate. The notice shows the share of medical costs to be paid.
Special medical care
People who hold a special medical care eligibility certificate or notice file at counter 23 on the South Annex 2nd floor.
