Overview
How to leave National Health Insurance by mail in Itabashi Ward. Send the eligibility change form with copies of the new eligibility certificate or notice, the originals if you still have them, and a copy of a government-issued photo ID to the NHI eligibility section. Write your address, the household head's name, your My Number and a daytime phone number on the copies. Registered mail is recommended; the ward processes the leaving once the documents arrive.
Leaving by mail
Everyone who leaves National Health Insurance needs a copy of the new eligibility certificate or eligibility notice, and the original if they still have it.
A person cannot leave National Health Insurance at their own choice.
My Number insurance card holders
Holders of a My Number insurance card may print the health insurance screen from Mynaportal and submit it instead of a copy of the certificate.
Documents that cannot be used
Employment insurance qualification notices, employment insurance cards, and health insurance and pension qualification and standard remuneration notices cannot be used for this procedure.
After the documents are sent
The documents contain personal information, so simple registered mail or specified-record mail is recommended; they must be taken to a post office. The leaving is processed as soon as the documents reach the ward office. If the premium changes, a recalculated premium is announced from mid-month of the month after the procedure is complete.
