Eligibility change form and consent form (mail enrolment)
Overview
How to join National Health Insurance by mail in Itabashi Ward. Send the eligibility change form, the consent form, a copy of the health insurance loss certificate and a government-issued photo ID to the NHI eligibility section. Households without a My Number insurance card receive an eligibility certificate; those with one receive an eligibility information notice. Registered mail is recommended, because the documents contain personal information.
Eligibility certificate or notice
Without a My Number insurance card, the ward sends the eligibility certificate by simple registered mail to the household head. People aged 70 to 74 also receive a senior recipient certificate. With a My Number insurance card, the ward sends the eligibility information notice by regular mail to the household head, and no senior recipient certificate is issued for people aged 70 to 74. The notice shows the share of medical costs paid by the insured person.
When the documents are sent back
The certificate or notice and the senior recipient certificate are sent about five office days after the ward receives the application. This does not apply when documents are incomplete or during the year-end and New Year holidays.
Sending the documents
The documents contain personal information, so simple registered mail or specified-record mail is recommended. Send them to Itabashi Ward Office, National Health Insurance Eligibility Section, postal code 173-8501; the street address can be left out.
