My wardsPower of attorney for National Health Insurance procedures
Overview
This page explains how to write a power of attorney when someone else files a National Health Insurance (NHI) procedure for you. The applicant writes the power of attorney personally. A table gives the wording to write for each procedure, such as an NHI certificate, joining, leaving, a reissue of certificates or an elderly recipient certificate, a premium reduction, or cancelling My Number insurance card registration. The phone number depends on the procedure.
Who writes it
