Use this form to declare that you qualify for a reduction of National Health Insurance (NHI) premiums because you left your job for an unavoidable reason, such as a company closure or restructuring. The form and a filling example are attached. If you file at the counter, bring the original Employment Insurance eligibility certificate or the original Employment Insurance eligibility notice.
