Overview
When it is hard for a person who needs consideration to see a doctor with a My Number insurance card, for example because a helper and a third person must accompany them to check eligibility, the ward issues an NHI eligibility certificate on application. Apply at the NHI and Pension Section counter, South Annex 2nd floor, counter 23, or at a citizen office. The application form and a filling example are attached.
When to apply
An eligibility certificate is issued on application when a My Number insurance card is hard to use, for example because a helper and a third person must accompany the person to confirm eligibility.
