Reissuing lost National Health Insurance certificates
Overview
If you lost your National Health Insurance eligibility certificate, eligibility notice or senior recipient card, you must apply for a reissue. You can apply online through My Number Portal, by mail, or at the counter. Online and mail applications need a copy of a photo ID document that confirms your identity. At the counter, bring photo ID and a My Number document. Registered mail is recommended, because the envelope holds personal information.
