Past insurance period and kigō/bangō
Overview
A person who was in national health insurance before can ask for the period of enrolment and the symbol and number (kigō and bangō) of the old card. This is given only at the counter of the ward office or a district office, after you show a document that proves your identity. For privacy, the ward does not give it by phone or email. The page also links to the list of identity documents needed for notifications.
