Identity documents for National Health Insurance notices

Overview

This guide lists the identity documents needed for National Health Insurance notices in Itabashi. One document from category A is enough, such as the My Number Card, a driver's licence, a passport or a residence card. Otherwise, two documents from category B are needed. A document showing your My Number is also needed, and an agent outside the household needs a proxy letter. Documents with an expiry date must still be valid.
