Application by post to issue or reissue NHI eligibility certificates

Overview

Use this form to apply by post for an NHI eligibility confirmation certificate or eligibility information notice, or for a reissue of an elderly recipient certificate. Send the form with a government-issued photo ID to the NHI Eligibility Unit at Itabashi Ward Office. Registered or special recorded mail is recommended. People without a My Number insurance card receive the certificate by registered mail, and people with one receive an eligibility information notice. Forms and a filling example are attached.

Certificates and notices

  • Without a My Number insurance card, the eligibility certificate is sent to the household head by registered mail. People aged 70 to 74 also receive an elderly recipient certificate.
  • With a My Number insurance card, the eligibility information notice is sent to the household head by regular mail. People aged 70 to 74 do not receive an elderly recipient certificate. The notice shows the share of costs to be paid.

Sending

The envelope contains personal information, so registered mail or special recorded delivery is recommended.

People who hold a special medical care certificate or notice file at counter 23 on the South Annex 2nd floor instead of by post.

Processing time

The certificate or notice, and any elderly recipient certificate, is sent about five office days after the ward receives the application. This does not include cases where documents are incomplete or where year-end and New Year holidays fall in the period.
