Premium reduction form for the period around childbirth
Overview
Use this form to claim a reduction of National Health Insurance (NHI) premiums for the period around childbirth, when an insured member gives birth. Attach the documents listed on the form, and bring a power of attorney if someone outside the household files it. The form and a filling example are attached. When to file
File this form when an insured member who gives birth qualifies for a reduction of NHI premiums for the period around childbirth.
Multiple births
For a multiple birth, the documents are needed for each child, so two Maternal and Child Health Handbooks are required for twins.
