Cityglot

Overview

The long-life health check for people enrolled in the late-stage elderly medical care system, to prevent lifestyle diseases and find illness early. It includes a medical interview, examination, blood and urine tests, an electrocardiogram, an eye check and a chest X-ray. The fee is 500 yen at the clinic counter, and it is free for households exempt from resident tax in the previous year. Bring the ticket, the examination form, your insurance card or eligibility certificate and the fee to a clinic commissioned by the ward. Some clinics need an appointment.

Who is not covered

  • Pregnant or postpartum women (who became pregnant or gave birth in the year)
  • People hospitalised for six months or more in a hospital or clinic
  • People in certain facilities under the law on securing medical care for the elderly, such as disability support facilities, the national institution Nozomi-en, elderly welfare homes, and paid or special nursing homes and care facilities
  • People who moved to the ward after already having a specific health check in the same year
  • People who already had a health check under social insurance in the same year

Tickets

Tickets are sent in June and July for the fiscal year. Keep the ticket safe and bring it to the check. The envelope contains the ticket, the examination form, the guide and the clinic list. A translation of the ticket and the form for foreign residents is available in the attached files. A lost ticket is reissued, which takes about one week. If you join the late-stage system during the year and no ticket arrives, contact the section.

Examination items

Medical interview, examination, body measurements, blood pressure, blood tests, urine tests, electrocardiogram, eye fundus examination, chest X-ray and others. Some items are done only for people who need them.

Fee and free check

The fee is 500 yen, paid at the clinic counter. Households exempt from resident tax in the previous year pay nothing. The ticket shows 'free' in the fee box, and bringing that ticket on the check day makes the check free. The lung, colorectal and prostate cancer checks taken on the same day are also free.

Free checks do not apply if another member of the same household is taxed, or if the household has not filed a tax return, because its tax status cannot be confirmed.

If you pay the fee without a ticket marked free, the fee cannot be refunded later, even if you are eligible. Apply for the free ticket before the check.

Applying for a free ticket

  • If you are eligible but the ticket does not show 'free', apply for a free ticket at least two weeks before the scheduled check date.
  • The application form is available from June at the specific health check counter on the 3rd floor of Setagaya Ward Office Building No. 2, or can be downloaded from this page.
  • The application form has a sample filled in.
