Dementia self-check sheet with your health check (Minato)

Overview

Residents aged 50 or over can ask for the dementia self-check sheet during their Minato ward health or cancer check. The sheet helps spot early signs of dementia, in cooperation with the Minato Medical Association. To take part, say that you want the dementia self-check sheet when you attend the check, and agree that the results may be shared with the Elderly Support Section and the Elderly Consultation Centre. The programme runs until 31 January 2027.
